It is important to note that the ASA’s concerns around the banned advertising were not focused on the safety of the Liquid BBL procedure itself. However, a report published by the government-approved organisation, Save Face, has shown that of over 2,000 medical professionals surveyed, 99% said they would not offer BBL Filler due to the associated risks. The medical professionals surveyed included qualified Plastic Surgeons, Doctors and Nurses.
Unfortunately, most Liquid BBL treatments in the UK are performed by unqualified people with no healthcare training or experience. This, of course, dramatically increases the risks to patients considering this procedure.
Between 2022 and 2024, Save Face received 479 complaints from patients who were unhappy with their results following Liquid BBL performed by non-medical practitioners. These included reports of medical complications such as infection, sepsis, abscesses, necrosis, and cellulitis. 91% of these patients required hospital treatment, 77% were in hospital for more than 48 hours, and 39% required surgical intervention.
Qualified medical professionals typically do not use Hyaluronic Acid based Dermal Fillers to augment the buttocks due to the high risks that may affect the procedure. The buttocks have a complicated and dense network of blood vessels, which can be permanently damaged, with life-threatening results, if mistakenly injected with Dermal Filler. Fortunately, there are alternatives to Liquid BBL, which are safer and have higher-quality results.

The safest surgical BBL procedure is performed with Fat Transfer. Fat is extracted from various areas of the body using Liposuction, processed to purify it, and injected into the buttocks. Crucially for safety, the fat must be injected beneath the skin, but above the muscle – a technique known as Superficial Gluteal Lipofilling. This dramatically reduces the risk of pulmonary embolism, where injected fat enters a blood vessel and travels to the lungs, causing a blockage. This is a serious and potentially fatal complication, which is typically associated with BBL surgery performed outside the UK. Previously, it was believed by some surgeons that injecting fat beneath the muscle reduced the amount of natural reabsorption that occurred. This has now been shown to be incorrect, and that injecting the fat deeper is simply more dangerous and doesn’t produce better results.
All surgical BBL procedures at Cadogan Clinic are performed in line with guidance from the British Association of Aesthetic Plastic Surgeons, published in 2021. This industry-transforming report was co-authored by Cadogan Clinic’s consultant surgeon, Mr. Omar Tillo. The risk of complications such as infection and nerve damage is slightly increased with the use of Buttock Implants due to the use of external implants and the complex nature of the network of blood vessels and nerves within the buttocks. In comparison, using the body’s own fat reserves reduces this, as the injected fat is recognised by the body.
