What is Gynaecological Fat Transfer?

Gynaecological Fat Transfer, also known as Labia Puffing, is a cosmetic gynaecology treatment used to treat a range of concerns affecting female genital tissues. Fat is extracted from various areas of the body using Liposuction before it is processed to isolate and obtain the adipose cells which can be used to regenerate tissues and add volume. The treated fat is then injected into the affected areas in a fat transfer procedure. The fat’s regenerative properties can be used to reconstruct and rejuvenate skin and tissue, addressing a number of issues including scarring, menopausal concerns and conditions arising after Radiotherapy treatment.

For many women, loss of volume around the labia, as well as concerns that have arisen due to hormonal fluctuations, pregnancy and childbirth, can lead to decreased confidence and physical discomfort. This can have a negative impact on an individual’s quality of life and may affect intimate relationships.

Gynaecological Fat Transfer vs Labia Fillers

Labia Puffing can be achieved through Fat Transfer or Dermal Fillers. Your surgeon will discuss the most suitable options to address your concerns during your consultation. 

 

Fat Transfer

Fat Transfer to the labia and vagina has several benefits:

  • Increased volume of the labia (also known as creating puffy labia)
  • Effective treatment for scarring and relief from the side effects of treatments such as Radiotherapy
  • Regenerative properties of fat naturally rebuild tissues that have weakened or deteriorated due to age or medical conditions
  • The results of fat grafting are permanent, although follow-up treatments can be carried out to enhance results
  • As the body’s natural fat reserves are used, there is no risk attached to a foreign object being placed in the body

 

Labia Fillers

Labia and Vagina Filler are an effective alternative for some patients with gynaecological concerns:

  • A non-surgical treatment with immediate effects and minimal downtime to increase the volume of the labia and create puffy vagina lips
  • Vagina Filler can be used to treat minor hormonal concerns
  • Dermal Vagina and Labia Fillers aren’t permanent, and repeat treatments will be required every 12-18 months
  • Although a specialist Labia and Vagina Filler product is used, it is crucial to understand the risks of an adverse reaction or issues such as migration
  • Labia Fillers and Vagina Filler don’t have any regenerative capacities, so aren’t able to rebuild broken-down tissues 

When considering Gynaecological Fat Transfer or Labia Puffing, it is essential to understand how different treatment options can address your concerns. Although thorough research is important, speaking with an experienced consultant is the best way to establish the most effective treatment plan to achieve your desired results. 


Labial Puff Before and After

During your consultation, your consultant will show you a selection of Labial Puff before and after images. These will be photos of patients who they have previously treated with similar concerns and desired outcomes to your own to give you a realistic comparison point. Your surgeon will also show you Labial Puff before and after results that have been achieved using both fat transfer and dermal filler to allow you to compare the results and make a final decision on the proper treatment options for your concerns. 

 


Which Conditions Can Gynaecological Fat Transfer Treat?

Gynaecological Fat Transfer is a highly versatile surgical technique and can be used to improve and address a number of conditions.

 

Lichen Sclerosus 

Lichen Sclerosus is an autoimmune condition which causes itchy white patches of skin around the genitals. It can affect people of any age, but is most common in women over the age of 50. The main symptoms are patches of skin which are:

  • Itchy 
  • Pale 
  • Have an unusual texture (such as very smooth or crinkled) 
  • Easily damaged and can crack or bleed

If left untreated, over time these patches can lead to tightness and irritation or the vulva shrinking. This can result in discomfort when urinating or during sexual intercourse. This can affect a woman’s quality of life and put strain on intimate relationships as the appearance of the patches, as well as discomfort can impact confidence and make people feel self-conscious. 

Lichen Sclerosus is not contagious and is caused by the immune system mistakenly attacking the skin, leading to damage. The reasons this occurs are not always understood.

To soften the patches, improve the appearance and tone of the skin, and also restore the shape of the vulva, if this has been affected, nano fat can be injected which has properties which are able to repair and rebuild the tissues.

 

Menopausal Concerns 

The menopause can lead to a number of physical concerns in the genital area for women. When combined with hormonal fluctuations and other changes they are experiencing, this can lead to a drop in confidence and loss in quality of life. By addressing these concerns, the menopause can feel less overwhelming, and confidence can be maintained.

 

Labia Atrophy 

Labia Atrophy is caused by the loss of oestrogen during menopause, which can result in dryness and inflammation in the labia majora and the receding of the labia minora. This can cause discomfort when urinating or during sexual intercourse and some women also experience self-consciousness, affecting intimate relationships.

These symptoms can be improved by injecting nano-fat into the labia area, rejuvenating the quality and appearance of the tissue, creating puffy labia. This reduces dryness and restores the shape of the labia, addressing the discomfort and increasing confidence.

 

Vaginal Dryness 

Also caused by a drop in oestrogen, vaginal dryness is a common concern associated with the menopause. It can be uncomfortable, and also cause pain or discomfort when urinating or during sexual intercourse. This can impact a woman’s quality of life, affecting intimate relationships and exacerbating emotional and mental health concerns which can also occur with the menopause.

By injecting nano-fat into the walls of the vagina, the tissue can be rejuvenated, and the dryness can be addressed, increasing comfort and addressing pain. The fat can also be injected into other areas of the internal and external genitalia, creating puffy vagina lips. This allows women to continue their normal activities and lifestyle, with confidence. 

 

Post-Childbirth 

Many women experience gynaecological issues following childbirth. Both Caesarean and vaginal delivery can lead to changes in the surrounding tissue which can result in a loss in confidence or potential discomfort during sexual intercourse or activities such as sport. Considering these issues occur alongside the life changing event of having a baby, this can exacerbate emotional distress as well as physical afflictions.

 

C-Section Scarring 

Following Caesarean Section surgery, a scar forms, where the incision is made. This is a horizontal line, usually 10-20 cm long running along the lower abdomen. In some instances, there may also be a vertical line, running up towards the belly button, forming a T shape.

This scar will start off as red and may look quite ‘angry’. Over time, it will fade and become white. However, in some cases as the scar heals, it remains raised (known as a hypertrophic scar). This can lead to the scarred area looking unappealing, resulting in women feeling self-conscious about this area. The scar tissue can also become hard and tight, known as scar fibrosis, which can feel uncomfortable and rub against clothing, causing further discomfort.

By injecting nano-fat into the scarred area, the reconstructive properties of the fat can rejuvenate and reconstruct the tissue. This results in a softer scar area and a more even skin tone. This improves comfort and quality of life as well as aesthetics and can improve confidence following C-Section surgery.

 

Episiotomy or Perineal Tear Scarring 

An Episiotomy is a medical procedure which is sometimes required during a vaginal birth. This involves a small incision being made between the vaginal opening and the perineum to make the vagina wider. This can be needed due to:

  • The baby’s heart rate increasing to a dangerous level (known as foetal distress) 
  • Having a breech birth 
  • Difficulty in giving birth 
  • Preventing a more serious perineal tear from occurring

A perineal tear occurs naturally between the vagina and perineum. Although the perineum is usually able to stretch effectively to allow the baby to pass through the vagina, it can sometimes become strained and tear. This is more common in women who haven’t given birth before. There are a few ways you can prevent perineal tears (which can also prevent the need for an Episiotomy) which your midwife will discuss with you during your pregnancy.

Following an Episiotomy or a perineal tear, the area will be closed with stitches to allow it to heal. For most women, the area heals well with no further issues. However, in some cases, scar tissue can overdevelop and harden, leading to discomfort during sexual intercourse or active exercise. This can cause rubbing in the area which can be painful.

For women experiencing these concerns, nano-fat can be injected into the area to rebuild the tissue and soften the scarring. This restores the texture of the perineal tissue and results in puffy vagina lips, reducing discomfort and rubbing to restore the quality of life.

Frequently Asked Questions

The length of time the results of Labia Puffing last depends on the techniques used during the procedure. Vagina Filler and Labia Fillers are non-permanent and require maintenance treatments every 12-18 months. It is important to remember that people metabolise filler at different rates, and several factors influence how often you may need to return to your consultant. 
 
In comparison, the effects of Fat Transfer Labia Puffing are permanent and do not require regular maintenance. Although Fat Transfer is a long-term solution, it does not stop the natural effects of ageing, and some patients do return for further fat injections 5-10 years after their initial procedure to counteract natural ageing. 

Puffy labia, and puffy vagina are both terms which have arisen from the development of the Labia Puffing procedure. Puffy labia and puffy vagina refer to the female genitals having a youthful volume, with smooth skin tone. Natural ageing, hormonal variations and some medical conditions can all have an impact on the labia and vagina, which may impact confidence, quality of life and sexual relationships. 

Following childbirth, you will need to wait until you are completely healed to safely undergo Gynaecological Fat Transfer or Labia Puffing. As this timeframe is different for every patient, and there are several influencing factors to consider, your consultant will advise when you can have your procedure. For most patients, this is between 6 and 12 months but will be impacted by the nature of your childbirth, whether you are breastfeeding, and if you are still under the care of another medical team.

The Risks of Gynaecological Fat Transfer

When considering Gynaecological Fat Transfer or any cosmetic surgery, it is vital to be aware of the potential risks and complications. Although Labia Fat Transfer results in a higher quality of life and reduces self-consciousness, there are risks that you need to consider and discuss with your surgeon.

  • Hematoma - This is the build-up of blood beneath the skin and will require treatment from your surgeon.
  • Poor wound healing - Healing complications or infection can affect the incision sites.
  • Seroma - This is the formation of a lump under the skin caused by buildup of fluid around the incision sites. This will require treatment from your surgeon.
  • Loss of sensation - This is usually temporary, but in rare instances can be long-term.

In the event of an unforeseen complication or less than optimal outcome, you may need a secondary surgery. This can occur due to factors beyond the surgeon’s control. Our skilled labiaplasty UK surgeons have performed this surgery numerous times before and endeavour to deliver the safest and highest quality results for our patients.

Your surgeon will also explain in your consultation that all surgical procedures carry a slight risk of:

  • Excessive bleeding
  • Allergic reaction to general anaesthetic
  • Deep vein thrombosis (blood clot in a vein)

Your surgeon will assess your personal risk for all of these factors alongside your general health and suitability for surgery. They will explain how these risks would be in the event they occur, and any steps you can take to reduce the risk of their occurrence.
