How Does a Deep Plane Facelift Differ From Other Techniques?

The tissues of the face and neck are made up of a number of layers. Whereas other Facelift techniques focus primarily on tightening the skin, the Deep Plane Facelift tightens the connective tissue known as the SMAS and also releases the underlying ligaments connecting the deeper tissues to the skin.

By releasing the ligaments in the deeper tissues of the face and neck, the surgeon is able to achieve more comprehensive results, as they are able to move the muscles and skin further than is possible in other Facelift procedures. Manipulating these ligaments also prevents the skin from looking overly stretched and unnatural – an unfavourable trait, sometimes associated with Facelift surgery.

As Deep Plane Facelift is a more invasive procedure than some other techniques, the incisions may be more extensive. Your surgeon will place their incisions around the ears, hairline, and occasionally extending into the scalp. As this technique is only performed by the most experienced surgeons, they will ensure the incisions are concealed in discrete areas where once they are healed, they will be as invisible as possible. 

What Other Procedures Can Deep Plane Facelift Be Combined With?

Deep Plane Face Lifting can be carried out as a standalone surgery, but is often combined with other procedures to enhance the final result. Popular procedures that are performed alongside Deep Plane Facelift include: 

  • Neck Lift – As a Deep Plane Facelift targets the lower face, a Neck Lift is a complimentary procedure which tightens and lifts the neck to give a balanced result

To understand what Deep Plane Face Lifting involves, it is important to explain the anatomy of the face and the layers of tissue that are being operated on. 

  • Skin: This is the uppermost layer of the facial tissue. The skin protects and the underlying tissues from environmental and mechanical damage. The skin has many small openings called pores which each contain a hair follicle and also allow for the removal of sweat and excess oil. 
  • Subcutaneous Tissue: This is the deepest layer of the skin and connects the skin to the underlying tissues. It is made up mostly of fat to insulate the body. This layer also contains a network of blood vessels and nerves, and connective tissue containing collagen and elastin. The base of the hair follicles is found at this level. 
  • Superficial Musculoaponeurotic System: Also known as the SMAS, this is a thin layer of connective tissue which lies beneath the subcutaneous tissue and above the deeper muscle. It plays a crucial role in facial expression connecting the muscles to the subcutaneous tissue and skin. As we age, the SMAS weakens and isn’t able to provide the same level of support to the skin. This contributes to the sagging of the skin and concerns such as jowls developing. 
  • Deep Plane: The deep plane of the face refers to the space between the SMAS layer and the deeper layers of muscle. This layer contains a network of ligaments which connect the deeper muscle and bone to the SMAS and overlying skin. This is the layer that is manipulated during a Deep Plane Facelift.
