What is Forehead Reduction

Forehead Reduction, also known as Hairline Lowering, is a surgical procedure to reduce the height of the forehead.

The size of the forehead is unique to individuals, and having a certain shape or size of forehead doesn’t necessarily determine whether you require Hairline Lowering surgery if it suits your appearance. Hairline Lowering provides an effective and long-term solution for those who feel their forehead is disproportionate to the rest of their face or doesn’t fit their identity.

Forehead Reduction surgery involves your surgeon making an incision concealed within your hairline along the upper forehead. A section of the forehead skin is removed, and the scalp is brought forward and attached to the new position, lowering the hairline and making the forehead smaller.

By reducing the size of the forehead, the proportions of the face are brought into balance, creating a harmonious and flattering appearance. This can improve concerns such as asymmetry.

Hairline Lowering or Forehead Reduction surgery has several aesthetic benefits:

  • Enhanced facial proportions: For those who feel their forehead is too large and affects the balance of their face, Hairline Lowering can bring the face back into proportion, stopping the forehead from overwhelming the face.
  • Smoothed forehead lines: When excess skin is removed from the upper forehead and the scalp is brought forward, lines and wrinkles may be reduced, and a smoother forehead may be achieved.

Why Do People Have Hairline Lowering Surgery?

People undergo Hairline Lowering or Forehead Reduction for many reasons. Hairline Lowering is a highly versatile procedure and can address a range of concerns, depending on the result you are hoping to achieve.

Ultimately, people have Hairline Lowering surgery because they are concerned about the appearance of their forehead or hairline. Forehead and hairline concerns have many causes and it is important to understand the factors affecting you to prepare for your consultation.

  • Receding or high hairline: A receding hairline is a common sign of ageing, mainly affecting men (although women can also experience hair loss). However, a thinning hairline can also occur from wearing overly tight or slicked-back hairstyles for long periods of time, putting pressure on the hair follicles and causing damage. A thinning or receding hairline can make the forehead appear more prominent and out of balance with the rest of the face.
  • Genetics: Genetics have a very strong influence on our appearance and facial proportions. In some cases, this may mean having a naturally prominent forehead. Although for some people, this will suit their facial proportions and not be a concern, for others, it may affect their confidence and self-image.
  • Gender dysphoria: Having a prominent forehead is often seen as a masculine facial feature. For anyone experiencing gender dysphoria, the appearance of their forehead may exacerbate this. Hairline Lowering is often carried out as part of Facial Feminisation Surgery or Non-Binary Facial Surgery resulting in a proportionate forehead and feminine hairline. Several facial surgeries are available to help address gender dysphoria.

Who Is Suitable for Forehead Reduction Surgery?

Your surgeon will assess your suitability for Hairline Lowering as part of your consultation. They will discuss your concerns and reasons for wanting Hairline Lowering, before examining your forehead and hairline. To decide whether you are suitable for surgery, they will consider a number of factors.

 

Forehead proportions and aesthetic goals

Your surgeon will take measurements of your upper face and forehead. Typically, to be considered proportionate, your hairline should be 6-7cm above your eyebrows. Lowering the hairline may benefit those whose hairline sits higher than this.

They will also discuss your desired results and what you hope to achieve from a Forehead Reduction procedure. If they feel you would benefit more from a different procedure, such as a Brow Lift or Hair Transplant, they will discuss this with you and explain how to seek an alternative option if they don’t offer it themselves. 

 

Hairline and scalp flexibility

As Hairline Lowering involves bringing your scalp forward, the flexibility of your scalp skin is important. Your surgeon must be able to manipulate your scalp enough to bring it forward comfortably. If you have limited scalp flexibility, additional techniques such as tissue expansion or grafting may be required to carry out the procedure.

 

Hairline density

The incision for Hairline Lowering usually sits within your hairline to conceal the scar, achieving a discrete result. Your surgeon will assess your hair’s thickness and growth pattern to ensure this is achievable and will make sure you understand if the scar may be visible.

 

Previous facial surgeries

It is important to make your surgeon aware of any previous facial surgeries you have had, such as a Facelift. Previous procedures may affect the skin’s elasticity and could impact how the procedure is carried out, or any additional techniques that may be required.

Hairline Lowering is a highly effective solution for balancing the face's proportions, reducing the forehead's size and creating a more even and dense hairline. When carried out by a specialist plastic surgeon, the results are natural and enhance the appearance, without looking obvious. 

Forehead reduction surgery is permanent, and the hairline will stay in its lowered position for life. However, Hairline Lowering cannot prevent further hair loss and if your hairline continues to recede, you may require further treatment such as a Hair Transplant. 

Although both Hairline Lowering and Hair Transplant can achieve the appearance of a thicker hairline, they use different techniques. Hairline Lowering moves the position of the hairline, achieving the appearance of greater hair density. A Hair Transplant involves taking donor hair follicles from another area with thicker hair and moving them to thinner areas.

What Are the Risks of Forehead Reduction
