What is Non-Binary Surgery?

Non-Binary Surgery is a combination of gender-affirming procedures for non binary individuals that seek to reshape the face and body to create an appearance better aligned to their gender identity.

Unlike binary transgender individuals who may seek to transition fully to male or female gender identities, non-binary individuals typically pursue a more personalized approach to surgical transition.  

The exact surgeries a non-binary patient chooses to undergo are therefore highly personal to them. Of course many non-binary people do not wish to change any physical attributes at all.

What are the Options for Non-Binary Surgery?

There are several procedures that non-binary patients can choose to undergo, depending on the manifestation of their gender dysphoria. 

 

Non-Binary Top Surgery

Non-Binary Top Surgery involves the reshaping of the chest contours. This usually involves the removal of breast tissue and changing the appearance of the nipples.

Non-Binary Top Surgery differs from FTM and MTF Top Surgery as each patient's desired results may vary more:

  • Some non-binary patients wish to have a flat chest that doesn’t have the typical definition associated with a male chest
  • Non-Binary Top Surgery may also involve the reduction of breast tissue without complete removal
  • Non-binary patients may ask for variations in the appearance of the nipples, either having their nipples removed or having the size altered
  • Depending on the desired result, the placement of the incision may vary, either sitting below the chest, closer to the muscle border, or across the centre of the chest

Non-Binary Top Surgery can be performed as a day-case surgery at Cadogan Clinic. 

 

Non-Binary Facial Surgeries

As with Facial Feminisation or Masculinisation Surgeries, Non-Binary Facial Surgery can be performed to alter the contours of the face, for patients who do not identify with the way they appear. Depending on the needs of the patient, there are several facial procedures which non-binary patients may choose to undergo:

Chin or Jawline Contour Surgery – These procedures reshape the chin and jawline, either creating a slimmer and softer, or a more angled shape. Depending on the desired result, this can either be achieved using facial implants, or by manipulating the bone structures in the jaw.

  • Chin Augmentation
  • Chin Reduction

Eye Area Surgery - Procedures which address the eye area can change the shape of the eyes, and address concerns such as hooded eyelids. Patients often opt to combine eye surgeries with other facial procedures to enhance the result.

  • Upper or Lower Blepharoplasty
  • Canthopexy or Canthoplasty

Forehead Surgery - Surgical procedures which address the forehead and upper face can change the shape and contours of the face. This can include altering the shape and size of the forehead or the positioning of the eyebrows.

  • Brow Lift
  • Forehead Reduction or Hairline Lowering

Facelift - For some non-binary patients who wish to change the shape or contours of their face, a Facelift may be suitable procedure. This may be the case for patients who are concerned about excess skin.

Rhinoplasty - For patients who experience gender dysphoria due to the appearance of their nose, a Rhinoplasty can be performed to alter the shape and size. There is a huge variety of techniques that can be used in Rhinoplasty surgery depending on the desired result and the patient’s concerns.

Fat Transfer - Facial Fat Transfer can be performed to give the face more volume and change its shape. This is often a popular choice as a permanent alternative to Dermal Fillers.

Non-Binary Body Surgery

There are several surgical procedures which non-binary patients may consider to alter the shape and contours of their body to suit their gender identity.

Liposuction – Fat extraction procedures can be used in various body areas to create the desired body shape and contours. Popular areas for Liposuction to be carried out for non-binary patients include:

  • Chest
  • Abdomen
  • Flanks
  • Thighs
  • Buttocks

Fat Transfer to Body – Fat can also be extracted using Liposuction and then processed to be injected into other areas of the body, adjusting the shape, volume and contours of different areas of the body. The areas where the fat can be injected depend on the desired results of the patient but may include:

  • Waist
  • Hips
  • Chest
  • Thighs

Body Contouring procedures are carried out as day-case surgeries at Cadogan Clinic. 

 

Non-Binary Bottom Surgery

Genital or Bottom Surgery reconstructs the genitalia. For non-binary patients, bottom surgery is a very personal decision and will relate to how the patient identifies with the genitalia they were born with. There are various surgical options available depending on the patient’s concerns.

Bottom surgery is not available at Cadogan Clinic. 

Hysterectomy, Oophorectomy & Vaginectomy – These are all procedures which remove different elements of the internal genitalia in patients who were assigned female at birth:

  • Hysterectomy removes the uterus
  • Oophorectomy removes the ovaries
  • Vaginectomy removes the vagina

There are several reasons why non-binary patients may wish to undergo these procedures. For patients who feel gender dysphoria around having periods or the presence of internal genitalia, these are effective procedures.

Orchiectomy, Scrotectomy & Penectomy – These are procedures which remove different elements of genitalia in patients who were assigned male at birth:

  • Orchiectomy removes the testicles
  • Scrotectomy removes the scrotum
  • Penectomy removes the penis

For patients who feel gender dysphoria due to external genitalia, these may be suitable procedures.

Who is Suitable for Non-Binary Surgery?

  • Your gender dysphoria diagnosis and your surgery referral. You will need two letters from a gender specialist psychologist to demonstrate this

Gender-affirming surgery is a personal choice for each person. While some non-binary people may choose to have procedures to reduce gender dysphoria, other patients may opt not to have surgery. Whether or not you choose to have Non-Binary Surgery will depend on how gender dysphoria affects your feelings about your body. 

The length of time it takes to recover following Non-Binary Surgery will depend on the procedures you have had and the techniques that have been used. Generally, most patients are mobile within 48 hours following surgery and fully recover within 6-8 weeks. Further Top Surgery recovery guidelines should be considered alongside any advice provided by your surgical team.

Although some patients will feel very confident about the surgical procedures they want to undergo to relieve gender dysphoria, other people may not be sure which procedures are suitable for them.
